Bentonville Parent Choice School

If you are interested in Parent Choice options, your student must have a Bentonville Schools student ID before this form can be completed. Families must first complete Admissions at their zoned school. Once Admission is complete, the Parent Choice application will be reviewed. A separate application must be submitted for each student. Notification of placement will be provided within 10 business days of submission. Please note that Parent Choice Schools utilize "Regional Busing", and some schools require uniforms/standardized dress.
