Important information from the nurse:

Immunization requirements: If your child is 11 years old as of 9/1/20, they MUST have the  Tdap booster vaccination. This is mandatory and proof must be presented to the school nurse by October 1, 2020 in order for your student to remain in school (this includes both blended and virtual students.) If you have any questions, please contact Nurse Tiffany at 479-254-5453 or email proof to

Health history: If you have not yet turned in your student's health history to the nurse, please do so ASAP. This ensures that the nurse is aware of any health concerns your student may have in case he/she presents to the nurse's office during school hours or has any medical issues during virtual learning.

One last thing...thank you for ensuring your student comes to school with a mask on each day, is practice social distancing and is washing hands accordingly. By us all taking an active part, we can prevent the spread of illness within our school and community and help keep us all SAFE here at Old High!!

Thank you, Nurse Tiffany
